Ozzy Osbourne Cause of Death Revealed

The death certificate lists additional ailments the rocker was facing.

The official cause of death for rock legend Ozzy Osbourne has been revealed just two weeks after his family announced his passing.

According to Osbourne's death certificate, obtained by The New York Times, he died of "out of hospital cardiac arrest" and "acute myocardial infarction." The report lists artery disease and Parkinson's disease as "joint causes" for his death.

The NYT reports Osbourne's daughter, Aimee, was the one to sign off on his death certificate.

It was July 22 when the Osbourne family shared his passing with the world, writing, "It is with more sadness than mere words can convey that we have to report that our beloved Ozzy Osbourne has passed away this morning. He was with his family and surrounded by love. We ask everyone to respect our family privacy at this time.”

Sharon Osbourne was joined by her children in a ceremony to honor the rock legend last week in the streets of Birmingham, where Ozzy grew up.

The family was understandably emotional during the event, relying on each other for support as they viewed a temporary memorial created by fans to honor Ozzy that was filled with photos, flowers and notes.

It was just two weeks before his death when Ozzy took the stage for one final time, reuniting with his Black Sabbath bandmates for the Back to the Beginning concert. Thousands of fans were in attendance for the show, while millions streamed it online around the world.

Ozzy belted out some of his most famous hits, reuniting on stage for the first time since 2005 with the core members of Black Sabbath. The concert also featured Metallica, Slayer and Guns N' Roses.

The Osbourne's were certainly appreciative during the memorial in Birmingham, briefly nodding to the crowd in thanks for the support as they struggled through tears.
